Big Leap Family Guidance Centers is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our expanding team in Hartford, Connecticut. This full-time position offers a remote work setting while operating within our vibrant outpatient clinic. Key Responsibilities\n

• Conduct thorough intakes on new patients that include diagnostic assessments and functional evaluations, followed by formulating tailored treatment plans.

• Provide focused therapeutic interventions, assisting patients in managing anxiety, depression, and other mental health conditions.
\n
• Collaborate closely with primary care providers to aid in recognizing and managing psychosocial issues, advising on which patients may benefit from specialized mental health services.
\n
• Initiate follow-up sessions to track patient progress, adjusting treatment plans as necessary to optimize outcomes.
\n
• Develop and implement relapse prevention plans to help patients maintain stability and prioritize long-term recovery.
\n
• Identify at-risk patients and strategize intervention plans to prevent worsening of mental health conditions.
\n
• Ensure comprehensive documentation of progress notes and treatment plans in accordance with agency and FQHC guidelines.
\n
• Empower patients by providing education and self-management skills that enhance engagement in their recovery journey.
\n
• Consult with clinic management and team members regarding behavioral health services evaluation and program improvement.
\n
• Assist patients in adhering to medical treatments prescribed by their primary care provider, including offering strategies for managing medication side effects.

• Adhere to the highest standards of ethical conduct within the clinical profession.

Patient Population / Client Focus\n

You will work with a diverse clientele, including children, adolescents, and adults from various backgrounds, each presenting with a unique set of challenges that may include mental illness, emotional distress, trauma, or life transitions. The counselor's role is pivotal in fostering a supportive environment conducive to healing and recovery while addressing the individual needs of each patient.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a closely related field is required.

• Active LPC (Licensed Professional Counselor) license in the state of Connecticut.

• In-depth knowledge of mental health assessments, treatment planning, and intervention methodologies.

• Understanding of developmental psychopathology and behavioral therapy theories.
\n
• Strong capability to evaluate patient needs and the effectiveness of provided services.

• Excellent interpersonal skills with the ability to build rapport with patients, families, and professional staff.

• Capacity to maintain comprehensive clinical records and prepare detailed reports.
\n
• Ability to work well in a multidisciplinary team setting.

Preferred Experience\n

• Experience working in outpatient or community health settings is highly preferred.

• Bi-lingual proficiency in English and Spanish is a plus, enhancing communication with a broader patient base.

Compensation and Benefits\n

The salary range for this position is competitive, offering between USD 87,000 and 105,000 annually, commensurate with experience and qualifications.
